Blood Sugar and Hormones: The Hidden Connection Behind Your Fatigue and Mood Swings

You've been told your hormones are the problem. Your estrogen is off. Your progesterone is low. Your cortisol is high. And so the conversation stays there — in the hormones — while the actual driver keeps running in the background, untouched.
For most of the women I work with, that driver is blood sugar. Not diabetes. Not even pre-diabetes. Just the kind of chronic, low-grade blood sugar instability that doesn't show up on a standard fasting glucose test but is quietly hijacking every hormonal system in the body, every single day.
Why Blood Sugar Is a Hormonal Issue
Blood sugar and hormones are not separate conversations. They are the same conversation. Every time your blood sugar spikes and crashes — which can happen multiple times a day depending on how you're eating — your body triggers a cascade of hormonal responses designed to bring glucose back into balance.
Insulin rises to clear glucose from the blood. Cortisol and adrenaline spike to raise it back up when it drops too low. These are survival mechanisms. They work. But when they're firing constantly — because blood sugar is unstable constantly — the downstream effects on your sex hormones, your thyroid, and your nervous system are significant and largely invisible on standard lab panels.
This is not a fringe theory. This is basic endocrinology that somehow never makes it into the conversation when a woman is told her hormones are "a little off."
The Three Mechanisms That Connect Blood Sugar to Your Hormones
1. The Pregnenolone Steal
Pregnenolone is the master precursor hormone — the raw material your body uses to make progesterone, estrogen, testosterone, DHEA, and cortisol. Under normal circumstances, your body distributes pregnenolone across all of these pathways based on what's needed.
But when cortisol demand is chronically elevated — which happens when blood sugar is chronically unstable — the body prioritizes cortisol production above everything else. It diverts pregnenolone away from the sex hormone pathways and toward cortisol synthesis. The result: low progesterone, disrupted estrogen metabolism, low libido, irregular cycles, and a hormonal picture that looks like a problem with the sex hormones when the actual problem is upstream in the stress response.
This is called the pregnenolone steal. It is one of the most common and most overlooked drivers of hormonal imbalance in women under chronic stress — and blood sugar instability is one of the most consistent triggers of that stress response.
2. Insulin's Effect on Estrogen and Testosterone
Elevated insulin — the kind that comes from a diet high in refined carbohydrates, frequent blood sugar spikes, or early insulin resistance — directly increases androgen production in the ovaries. This is the primary mechanism behind the hormonal pattern in PCOS: high insulin drives high androgens, which disrupts ovulation, which disrupts progesterone production, which creates estrogen dominance.
But you don't have to have PCOS for this to be relevant. Any woman with chronically elevated insulin is running a hormonal environment that favors androgen excess and estrogen dominance. The symptoms — acne, hair thinning, irregular cycles, weight gain around the midsection, mood instability in the second half of the cycle — are often attributed to the hormones themselves when the driver is insulin.
High insulin also suppresses sex hormone binding globulin (SHBG), the protein that carries estrogen and testosterone through the bloodstream. When SHBG is low, more of these hormones are "free" and biologically active — which sounds like a good thing until you realize it means your tissues are being flooded with hormonal signals your body isn't equipped to process cleanly.
3. Blood Sugar, Cortisol, and Thyroid Suppression
The thyroid is exquisitely sensitive to stress. When cortisol is chronically elevated — driven in part by blood sugar instability — it suppresses the conversion of T4 (the inactive thyroid hormone) to T3 (the active form your cells actually use). This is why so many women have "normal" TSH and T4 levels but still experience every symptom of hypothyroidism: fatigue, cold intolerance, hair loss, constipation, brain fog, and weight that won't move regardless of how little they eat.
The thyroid is not broken. The conversion pathway is impaired — by cortisol, by inflammation, by nutrient deficiencies that blood sugar dysregulation contributes to. Treating the thyroid in isolation, without addressing the blood sugar and cortisol picture driving the suppression, is why so many women feel only marginally better on thyroid medication.
Fix the blood sugar. Reduce the cortisol burden. The thyroid often starts functioning better on its own — sometimes dramatically so.
What Blood Sugar Instability Actually Looks Like
You don't need a diabetes diagnosis to have blood sugar dysregulation. The signs are subtler and far more common than most people realize:
You wake up tired even after a full night of sleep. You need caffeine to function before noon. You get irritable, shaky, or mentally foggy when you haven't eaten for a few hours. You crash in the afternoon between 2 and 4pm. You get a second wind at 9 or 10pm and struggle to fall asleep. You wake up at 3am with your mind racing. You crave sugar or carbohydrates in the evening. You feel worse, not better, after eating a high-carb meal.
These are not personality traits. They are not signs of weakness or poor willpower. They are physiological signals that your blood sugar is running on a roller coaster — and that roller coaster is pulling your hormones along for the ride.
The One Lever That Changes Everything
Before we run any testing, before we look at any supplements, before we talk about anything else — I want you to eat protein within an hour of waking up. Not coffee. Not a piece of fruit. Protein. Eggs, meat, Greek yogurt, a protein shake. Something that anchors your blood sugar from the start of the day and prevents the cortisol spike that comes from waking up in a fasted, glucose-depleted state.
Then I want you to build every meal around a protein anchor — at least 25 to 30 grams — with fat and fiber alongside it to slow glucose absorption. Eat every four to five hours during the day. Don't skip meals. Don't do extended fasting if your hormones are already dysregulated. Give your body the signal that food is available and the threat response can stand down.
For many women, this single change — protein first, stable blood sugar across the day — produces noticeable improvements in energy, mood, sleep quality, and cycle regularity within two to three weeks. Not because it fixes everything. Because it removes one of the most consistent triggers of hormonal disruption from the equation, and gives the rest of the system a chance to recalibrate.
How We Assess Blood Sugar at Mind Body Restored
A fasting glucose test tells us almost nothing useful about blood sugar regulation in women who don't yet have diabetes. What I look at instead is fasting insulin — which reveals insulin resistance years before glucose becomes abnormal — alongside hemoglobin A1c, which reflects average blood sugar over the past three months.
I also look at the full hormonal picture in context: cortisol patterns, sex hormone levels, thyroid conversion markers, and inflammatory markers. Blood sugar dysregulation rarely travels alone. It shows up alongside elevated cortisol, suppressed progesterone, disrupted thyroid conversion, and elevated inflammatory markers — a cluster that standard care addresses one piece at a time, if at all.
When we address the blood sugar piece, the rest of the hormonal picture often shifts more than any individual hormone intervention would have achieved. This is root-cause medicine. This is what it looks like when you stop treating the downstream effects and start addressing what's actually driving them.
